Kindergarten Registration
Kindergarten Registration will be held Monday, May 1st 2023
for the 2023-2024 School year.

Children planning to attend Kindergarten next fall must:

be five years old on or before September 15, 2023

have a birth certificated (Original) or proof of birth

have an official record of current immunizations: Within 30 days of entering Kindergarten, each child must have 5 DPT's, 4 OPV's, 2 MMR's and the Hepatitis B series. At least one DPT and One OPV or IPV must have been administered on or after 12 months of age and the 2nd dose by school entrance date. Please call Public Health at 789-9203 or your child's physician if you have question regarding immunizations.
All parents of children planning to attend Kindergarten next fall must:
- bring their child's birth certificate or proof of birth, official record of current immunizations to Kindergarten Registration
Parents who have a child with special needs and who have not been involved with the Lincoln-Uinta Child Development Association should contact Matt Williams, at 789-7571, extension 1041.
